STUDENT
HEALTH AND UIC HEALTH INSURANCE FEES
As
part of your College of Medicine-Rockford student fees, each
semester you are charged two health related fees: student
health and health insurance. Per fall and spring semesters,
the student health fee is $88 (student health services and
mental health) and the UIC Student Insurance fee is $315.
Summer semester fees are yet to be determined but will be
approximately $59 and $207, respectively. To receive student
health and counseling services, you must be a registered student
who has paid the student health fee and has insurance.
STUDENT
HEALTH SERVICES
All
seven of the College of Medicine-Rockford Clinics will provide
student health services (described below) to registered students
using the funds collected from the Student Health Fee/Student
Counseling Fee and insurance reimbursement. (Expenses for
the academic year will be reviewed each semester. Services
provided are subject to change.) As is the case
with all patient records at our clinics, student medical records
are kept confidential.

As
a benefit to UICOM-R registered students who use services
at our UICOM-R clinics, the deductible will be waived. Generally
there will be no out of pocket expenses for services received
at a UICOM-R clinic.
Qualification:
- Must be a current
student or an immediate family member (Spouse and dependent
children 22 years and under).
- It is mandatory
that all students carry health insurance. For immediate
family members without insurance, there will be a flat discount
of 25% on all services received from our UICOM-R clinics.
- It is mandatory
that the providers keep a chart and records for every student
or family member at every visit.
- NOTE: Discounts
will not be provided for services considered cosmetic or
non-medical in nature. Medical supplies are excluded from
this policy.

you receive a billing notice and are unsure of the contents,
please bring the notice as soon as possible to Allena Fortson,

STUDENT
HEALTH INSURANCE
Most
health insurance policies cover injury and illness but do
not provide a benefit for preventive care. It is recommended
that you know your health insurance coverage and benefits.
It is mandatory that all students have health insurance.
Carry your health insurance card in your wallet
at all times.
Most
students enroll in the health insurance provided by the university,
UIC Student Health Insurance. However, some elect to obtain
individual coverage or are covered by a family member’s policy.
Those who are not on the UIC Student Health Insurance must
submit a form by a mandated date to our Rockford Registrar,
Allena Fortson, (S-315, 395-5581) which indicates exemption
to the university insurance. If you do not submit this form,
you will automatically be billed for the UIC Student Health
Insurance.

LAPSE
IN UIC STUDENT HEALTH INSURANCE
While
not registered as a student you DO NOT have the university
health insurance, so it is strongly recommended that you acquire
other coverage. If you were on the UIC Student Health Insurance
policy for at least 6 consecutive months, prior to a lapse
in the UIC Student Health Insurance coverage (and are ineligible
for other insurance coverage such as Medicare), you can purchase
an extension of the UIC Student Health Insurance policy for
up to 6 months. However, there will be charges
for all student health services not covered by your health
insurance. Talk with our Rockford Registrar,
Allena Fortson (S-315, 395-5581).
HEALTH
RELATED ISSUES FOR OUT-OF-ROCKFORD ELECTIVES
Students
who wish to do an out-of-city elective should contact the
other school to inquire about health -
related requirements. Immunizations and/or titers
that are required by other institutions, but not by UIC-Rockford,
are the responsibility of the individual student.
If
an out-of-city rotation requires immunizations, titers, or
both, you can get them done at the University Family Health
Center. However, payment will be YOUR
responsibility. Most health insurance policies do not include
these in their coverage.
To
facilitate your requests, we ask that you do the following:
Identify the date by which you would like the
results. Next contact Nancy Peterson ( nanpeter@uic.edu
or 972-1000) to discuss the issue and determine the time
frame needed for obtaining immunization and/or titer materials
in order to have the results by the needed date.

one week prior to your desired date
to receive results. State the immunizations and/or titers
required and for what rotation and site.
After sending the email, you may go the University
Family Health Center. You do not need an appointment for
a lab. The lab hours are 8:30-5:00, Monday through Friday.
You do not need any appointment as long as you have
contacted Nancy Peterson.
When the results are received by the University
Family Health Center, Nancy Peterson will forward a copy to
Allena for your student records. Allena will also put a
copy of the results in your mailbox.
Students
who wish to do electives in other countries must contact the
Office of Medical Education and Evaluation, (S-319, 395-5590),
as early as possible to receive information concerning health
related issues. Students will be required to obtain appropriate
prophylaxis and purchase evacuation insurance.
PRESCRIPTIVE
DRUGS
Check your health insurance policy regarding
benefits and procedures.
UIC Student Health Insurance: 100% allowable
charges/$2500 maximum per policy year with a deductible ($10
for generic and $20 for non-generic). For exclusions, check
your health insurance policy. Students can go to any pharmacy.
You pay the bill and send the receipt for reimbursement
(see Claim Procedure section in your brochure).
The Oral Contraceptive Discount for registered
medical students requires a prescription from an “authorized”
UICOM-Rockford Provider at one of our UICOM-Rockford Clinics.
Payment (cost of medication, shipping and handling) will
be made directly by the student to UIC Eye and Ear Infirmary
(EEI) pharmacy and the prescription will be mailed directly
to your residence. You and your UIC physician will complete
the required form for order and payment. A credit card is
preferred for payment and faster processing of the prescription.
MENTAL
HEALTH/COUNSELING SERVICES
It is recommended, but not required, that you
initially consult with Dr. Kashima, (S-309, 395-5630) or Kathy
Norman, (S-302, 395-5629), because there may be several options
for resolving your concerns.
Check your health insurance policy regarding
mental health benefits.
UIC Student Health Insurance: 50% allowable
charges/$50 per day/52 days maximum for providers
in and out of the CCN network .
As a benefit to students, the UIC-Rockford
College of Medicine University Psychiatric Services will waive
your insurance co-payment for up to six sessions per academic
year if you have UIC Student Health Insurance. After the
six sessions, the student’s insurance should still apply (up
to $50 per day up to 52 visits per year). The $45 co-payment
will be discounted 50% so the student will pay $22.50 per
visit. When the student’s insurance does not provide coverage,
it remains the student’s responsibility for payment. The
list of providers excludes Drs.
Garcia and Winegarden. Other policies will be reviewed before
payment is established. Call Nancy Vandervort, Dr. Slack’s
assistant, at 395-5874 for an appointment.
If you choose a non-UIC Rockford College of
Medicine Mental Health provider, your student health fee will
cover your co-payment for up to six sessions per academic
year. You must pay the deductible.
In all cases, bring your mental health/counseling
bills to Student and Alumni Affairs, Allena Fortson, (S-315,
395-5581).
There are several community mental health professionals
in Rockford. Students have had both positive and negative
experiences. Evaluate your mental health professionals just
as you would any health professional. Ensure that you have
a positive working relationship to obtain the most out of
the experience.
FAMILY
HEALTH SERVICES
For
insured family members of registered students, injury and
illness services provided at the UICOM-Rockford Clinics will
waive the balances left unpaid by insurance.
This
benefit does not apply to Preventive/Wellness care.
